Consultation

The initial consultation is a brief in-person or telehealth video session designed to explore your interest in natural medicine services and determine whether this work may be an appropriate fit. During this time, we will review your goals, discuss eligibility considerations, and walk through the required steps for screening, paperwork, and payment through the Healing Center. This conversation also provides an opportunity to ask questions about the process, preparation, and what to expect before deciding how to move forward.

Forms & Questionnaires

The screening process includes completion of a series of standardized questionnaires and required intake forms to ensure safety, appropriateness, and informed participation. Clients will be asked to complete baseline measures and a safety assessment to help guide clinical screening and readiness. In addition, several required forms will be provided through the Healing Center, including facilitator disclosures, demographic information, informed consent documents, safety planning materials, transportation planning, privacy and data use notices, and other regulatory attestations required prior to participation. These materials support informed consent, risk mitigation, and compliance with DORA regulations, and are reviewed as part of the structured preparation process before any facilitation session occurs.
